What tadalafil is
Tadalafil is a prescription medicine for erectile dysfunction, approved in 2003. It is the active ingredient in Cialis. Like sildenafil, it has been in normal use for two decades and is one of the most prescribed medicines of its type in the world.

Same target, different pace
Tadalafil blocks the same enzyme sildenafil blocks. The difference is timing. Tadalafil takes longer to build up in your blood, and then it hangs around far longer before your body clears it out. That is a chemistry difference, not a strength difference.
So on its own, tadalafil covers a long stretch but wants a head start. Sildenafil gives you the head start. Neither one fixes the other's weak point on its own, which is the whole reason both are in the same tablet.
What "up to 36 hours" actually means
It does not mean an erection for 36 hours. If that ever happened it would be a medical emergency. It means that for up to 36 hours after the dose, your body responds more easily when you are aroused. Nothing happens on its own in between.
The practical benefit is that you stop scheduling. You do not have to decide at 7pm whether something might happen at 11pm. For a lot of men that is the change that matters more than anything about the erection itself.

Side effects
The common ones are the same family as sildenafil's: headache, flushing, a blocked nose, indigestion, feeling light-headed. Tadalafil is a little more likely to cause back ache or muscle ache, usually 12 to 24 hours after the dose, and it usually passes on its own.
Because it stays in your body longer, a side effect from tadalafil can also last longer than the same side effect from sildenafil. That is worth knowing before your first dose so it does not catch you out.
Emergency symptoms
Chest pain during sex, sudden vision or hearing loss, or an erection lasting more than four hours. Call 911 or get to an emergency department — do not wait to see if it passes.
Who should not take it
The exclusions are the same as for sildenafil and for the same reason — both drugs work the same way. Nitrates of any kind are an absolute no. So is riociguat. Recent heart attack or stroke, unstable angina, uncontrolled blood pressure, severe liver or kidney problems and a history of NAION all rule it out too.
If you take an alpha-blocker for blood pressure or for prostate symptoms, say so on the health form. It does not always rule you out, but the doctor may want the doses spaced apart.
Common questions
Tadalafil is the active medicine in Cialis. Cialis is Eli Lilly's brand name for it. HardRx contains tadalafil but is not a Lilly product and has no connection to the company.
The medicine stays active in your body that long for many men. It makes a response easier during that window; it does not produce anything on its own.
Back and muscle ache is a recognised tadalafil side effect, usually 12 to 24 hours after the dose. It normally passes on its own. Tell the prescriber if it keeps happening.
